Yearly Physical and HRA
Taking control of your wellbeing can start with establishing a baseline of your physical health, a biometric screening, physical and health risk assessment (HRA), or short medical questionnaire. By doing so you can identify unknown chronic conditions and encourage behavioral changes to start a journey to more complete wellbeing. The MyLyfe Wellness staff will come to your place of business to conduct an HRA and a lipid blood panel or physical, all in about 15 minutes with minimal disruption to your work schedule. Participants will leave with their results from the screening or physical, and the organization’s results will be aggregated and presented as a snapshot of the organization’s overall wellness to be used in making the most appropriate healthcare decisions for your employees.
Onsite or Near Site Clinic
Countless hours of production, and hours of paid time off are squandered every year by employees leaving work to wait at doctor’s office to find out if they are sick. By allowing MyLyfe Wellness to bring a medical provider to you, you can minimize those lost hours by letting employees visit a clinic without leaving work. Based on your employee population, MyLyfe Wellness will regularly visit you with a mobile clinic or establish a clinic with set hours inside your facility. If you do not think you have enough employees at your site to justify an onsite clinic, a near-site clinic may be shared among neighboring businesses.
Chronic Disease Management

MyLyfe Wellness will benefit employees who live with chronic illnesses by keeping them informed about best practice methods for managing their conditions and by delivering their treatment medications to the job site. According to the non-profit Foundation for a Healthy Kentucky, nearly 70 percent of Kentuckians live with asthma, diabetes or hypertension. Although these conditions are treatable, they make up a large percentage of costly insurance claims for emergency room visits and inpatient hospital stays. Since 1997 the American Pharmacist Association Foundation has shown a measurable reduction in health care related costs associated with chronic conditions by encouraging those with a chronic disease to participate in educational sessions to learn more about their condition and the medications prescribed to manage it.
Fully Staffed Clinic
According to a 2016 University of Kentucky College of Medicine article, there are 2,696 practicing primary care physicians serving the Commonwealth’s 4.4 million residents. Long wait times and delayed appointments have become common when seeking medical treatment. However, MyLyfe Wellness has a solution for those obstacles to health care. A fully equipped Federally Qualified Health Center inside the walls of your facility is available to your employees and their families operating during the hours you choose. Medical staff would be on hand to treat most common illnesses and work-related injuries, test for flu and many other issues for which you would typically visit a primary care clinic.
Health Navigator
Navigating the world of health care can be time consuming and confusing, but with one call or email to your organization’s very own live Health Navigator, you can be seen at one of Cumberland Family Medical Center’s clinics or area specialists. A MyLyfe Wellness Health Navigator can schedule your medical appointments based on the care that is most appropriate for you and your schedule, assist with insurance questions and help you Live to Your Fullest Extent!
Al la Carte Options:
Yearly Physicals for Commercial Drivers: Make sure your fleet stays on the road by scheduling drivers’ yearly physicals. MyLyfe Wellness can conduct required DOT physicals onsite, or drivers can obtain a physical seven days a week, from 8:00am-8:00pm without an appointment at First Choice Immediate Care located at Cumberland Parkway Exit 47 in Columbia, KY. Truck parking is available.
Immunization Clinics: Keep your team healthy and productive year-round by scheduling an onsite vaccination clinic. A variety of immunization clinics are available; please let us know your needs.
Educational Opportunities: MyLyfe Wellness can facilitate educational opportunities relevant to your workforce based on results from our Health Risk Assessment. Topics are specific to your needs and may include smoking cessation, nutrition, and exercise.
Industry Specific Testing: Blood, skin, saliva, and urine tests specific to a job function or industry are available.
